Hospice admission guidelines for renal disease

Acute Renal Failure — Criteria 1 and either 2 or 3

1. Dialysis / Transplant Status

Not pursuing dialysis or a renal transplant, or has ceased dialysis treatment.

2. Creatinine Clearance

  • <10 cc/min (<15 for diabetics)
  • Or <15 cc/min (<20 diabetics) with CHF

3. Serum Creatinine Levels

Greater than 8.0 mg/dl (greater than 6.0 mg/dl for diabetics).

Chronic Renal Failure — Criteria 1 and either 2 or 3

1. Dialysis / Transplant Status

Not seeking dialysis or a renal transplant, or discontinuing dialysis.

2 & 3. Creatinine Clearance / Serum Creatinine

  • Clearance <10 cc/min (<15 diabetics), or <15 (<20 diabetics) with CHF
  • Serum creatinine >8.0 mg/dl (>6.0 diabetics)
