At certification/recertification, the patient has already received the best possible treatment, is not a surgical candidate, or has declined the procedure. (Not on vasodilators due to valid medical reasons such as hypotension or kidney disease.)
Severe heart failure or angina symptoms even at rest, with discomfort at any physical activity. Severe CHF may be documented by an ejection fraction of 20% or lower (not strictly required if unavailable).
